Text Box: HEALTH AND DENTAL INSURANCE - Blue Cross Blue Shield  Eligibility for enrollment is thirty (30) days from the date of full-time employment.  Coverage will begin on the first day of the month following thirty days of employment.
It is important to understand the type of medical coverage available and to decide if the plan is the most appropriate for your individual needs.  Premiums are pre-taxed payroll deductions every two weeks.
If an employee chooses not to participate in CHA’s Health/Dental Insurance Plan, he/she will not become eligible again for enrollment until March of the following year.  Exceptions to this rule can only be made if the employee has a qualifying reason, such as:  the birth or legal adoption of a child, a marriage, or loss of benefits through another plan.
An employee may not drop health/dental coverage until March of the following year.  Exceptions to this rule can be made if the employee has a qualifying reason, such as: death, divorce, a child becoming of age, or termination of employment.
NOTE: The employee must notify Human Resources within 30 days of the qualifying  reason to participate and/or make changes on their health/dental coverage.
Long Term Disability Insurance – Mutual of Omaha    Eligibility for enrollment is thirty (30) days from the date of full-time employment.  Coverage is for the employee only and will begin on the first day of the month following thirty days of employment. CHA pays the monthly premium. (No cost to the employee.)
ADDITIONAL OPTIONAL INSURANCE – Colonial Life   Eligibility for enrollment is thirty (30) days from the date of full-time employment.  If interested, employees may contact the HR Department. Additional insurance (cancer, disability, accident, etc.) will be offered through payroll deduction. The employee will be responsible for these premiums and coverage will begin on the first day of the month following thirty days of employment. These plans do not replace/substitute health insurance.
COBRA – Blue Cross Blue Shield/US Admin  Within sixty (60) days upon termination of employment, former employees may sign up for COBRA coverage. US Admin will be notified and will send you information and forms to complete.  The former employee must pay all premiums.  (Only employee who had Health/Dental coverage and their covered dependents are eligible.)
MEDICAL & DEPENDENT CARE FLEXIBLE BENEFITS – Blue Cross Blue Shield   Eligibility for enrollment is thirty (30) days from the date of full-time employment and will be offered through payroll deduction on a pre-tax basis.
